Every patient has to fill out informational forms to assist the doctor in providing the best possible eye care. If you are a new patient at Beaver Eye Care, we ask that you arrive a few minutes early to fill out the necessary forms. We also ask that you bring your insurance card along if you plan on processing your treatment through insurance. Patient Medical History Questionnaire
We accept all major medical insurance companies. The VISION insurance plans that we accept include VSP, Metlife VSP, Blue Cross Blue Shield Hawk-I, and Medicaid. We are out of network for EyeMed and Avesis, but we have the forms for these companies available at our office.
